Bronchial Stenosis in Patient with Wegener¡¯s Granulomatosis

Abstract
The patient, 70 year-old woman, presented with over 1 year history of nonproductive cough and progressive dyspnea. She was diagnosed at our clinic as Wegener¡¯s granulomatosis (WG) with features of granulomatous inflammation in lung parenchyme, pauci-immune glomerulonephritis and positive antiproteinase-3 ANCA (antineutrophil cytoplasmic antibody). While she had been receiving glucocorticoid, azathioprine and respiratory antibiotics, her respiratory symptom had gradually worsened. Chest computed tomography revealed mid-bronchial luminal narrowing, multiple irregular cavitary nodules in the peripheral portion of both lungs and a wedge-shaped consolidation. She was diagnosed as bronchial involvement in WG and was treated with bronchial bougination. Bronchial stenosis is rare and occurs independently of other features of active WG. These can be a life-threatening manifestation of disease, so prompt diagnosis is essential. Since the presenting symptoms of bronchial stenosis can be nonspecific, any patient with WG who has increasing dyspnea, voice change or nonproductive cough should be considered in the differential diagnosis.
